Nine-Year-Old becomes first in UK to undergo groundbreaking leg lengthening surgery
A nine-year-old boy has made medical history as the first child in the UK to receive a pioneering leg-lengthening operation.
Alfie Phillips, from Northampton, was born with fibular hemimelia — a rare condition that affects fewer than one in 40,000 births and caused his right leg to develop shorter than his left by more than an inch.
Specialists at Alder Hey Children’s Hospital successfully carried out the innovative procedure, enabling Alfie to gain 3cm (1.2in) in length.

Reflecting on the experience, Alfie said he felt both excited and nervous about being the first child in the country to undergo the surgery. Nearly a year later, he says he is back to “running around as normal” and enjoying sports such as basketball and trampolining.
How the procedure works
The technique, developed in the United States, involves placing a motorised telescopic nail along the surface of the femur — the long thigh bone. Unlike previous internal lengthening nails used in adults, this method is safe for children as it avoids damaging growth plates.
Consultant orthopaedic surgeon Nick Peterson explained that internal lengthening offers a less painful and overall better experience compared to older methods.
Before this breakthrough, children like Alfie would have required an external fixator — a bulky frame attached outside the leg — to gradually stretch the bone.

After the nail was implanted in March 2025, Alfie spent less than a week in hospital. A magnetic device was then applied to his leg three times daily for a month, slowly extending the nail by about 1mm per day. As the bone separated, his body naturally generated new bone tissue to fill the gap.
The lengthening process took approximately six weeks, followed by regular physiotherapy and medical reviews. The nail was removed three to four months later.
A remarkable recovery
Alfie’s mother, Laura Ducker, described his recovery as extraordinary, saying he “coped magnificently” and was eager to return to school.
Now fully active, Alfie says the operation has transformed his abilities. “I can do a lot of things I couldn’t do before, like skipping. I’m better at basketball and I can do trampolining too,” he said jokingly adding that he is now taller than one of his friends.

Doctors had initially measured a 4cm difference between Alfie’s legs and predicted it could have reached 6cm by adulthood without treatment. While he may require further lengthening of his shin bone in the future, Peterson described his recovery as “remarkable” and far superior to what would have been possible with older techniques.
Since Alfie’s surgery, Alder Hey has performed the same procedure on three other children, with additional specialist centres across the UK preparing to adopt the method — a development doctors believe could transform treatment for young patients with similar conditions.
